Our in-house therapy team is led by Area Directors of Therapy Services; our directors require therapists to have experience in orthopedics, home health, home exercise programs, special precautions, and manual therapy. Each physical therapists’ competencies are signed off by the Area Director of Therapy Services. Most of our therapists are Doctors of Physical Therapy with years of experience treating orthopedic and spine patients.

Outpatient therapy is a vital part of the recovery process, focused on maximizing your strength, endurance, and balance with practical activities and even your favorite hobbies. GrandCare provides outpatient therapy in the comfort and convenience of your home.
Orthopedics is a branch of medicine focusing on the care of the musculoskeletal system made up of muscles, bones, joints, ligaments and tendons. The most common orthopedic surgeries include:

Home health services are prescribed by a physician when a patient is homebound following a surgery or medical setback. Home health therapy exercises are less advanced and are focused on restoring functional mobility to be able to do activities of daily living (transfer, bathing, dressing, grooming) without any falls or injuries. These exercises are focused on restoring the patient’s range of motion (ROM) and ambulation so that they are no longer homebound and can transition to outpatient therapy.
Outpatient therapy services are provided after home health is complete. Patients are no longer considered homebound and are a few weeks post-operative. These exercises are more advanced and are focused on restoring and maintaining strength, endurance, flexibility, stability and balance.
Outpatient therapy is covered by most Medicare, HMO and Commercial insurance plans. In some cases, there may be a co-pay.

Outpatient therapy can be beneficial to individuals who are recovering from an injury, surgery or illness. Outpatient therapy is a course of treatment that supports a patient’s recovery and rehabilitation and can be delivered in a clinic setting or at home. A physician will provide a plan of care for patients and will decide whether outpatient therapy is right for each individual patient. Some patients may benefit from receiving outpatient therapy in the home setting, while others may benefit more from a clinic environment. Finding the right environment for rehabilitation can shape the recovery for an individual. The patient’s physician will decide which environment is best for each individual patient’s needs.
We strive to have the same therapists work with the patient for each visit throughout the course of care. If there are scheduling conflicts, sometimes this may not be possible, and it will be communicated in advance.
Yes. All patients have the right to control the care that is provided to them. This includes choosing the provider that renders the care. Your choice must be honored. You can ask your physician or hospital discharge planner about home health services and inform them that you would like to choose GrandCare if you prefer our services.
