GrandCare Assisted Living Services is a Care Coordination Agency (CCA) for the Assisted Living Waiver (ALW) program. We have been coordinating care for people in the Southern California region since 2008. Our team specializes in the enrollment process and is dedicated to helping participants find placement in a local assisted living facility that can best meet their care needs.

Here are the most frequently asked questions and answers we get.

The Assisted Living Waiver (ALW) is a waiver that was created by legislation to provide assisted living care services as a Medi-Cal benefit. Visit the Department of Health Care Services to learn more.

Yes. As long as you provide us, your Care Coordination Agency (CCA), with your social security number and date of birth we can add you to the waitlist. You will need to enroll in Medi-Cal by the time we have to assess.

No, this program is not free. The Assisted Living Waiver (ALW) Program requires that you use your social security and supplemental security income to pay for room and board. For up-to-date information on the SSI, please visit the Social Security Administration website.

Room & Board cost is determined each year based on the federal Social Security Administration (SSA) published maximum SSI benefits available to beneficiaries in different living arrangements. Most ALW participants use their Social Security Income/State Supplementary Payment (SSI/SSP) to pay for rent. Please contact your care coordinator to identify the current rate.

Yes. Throughout the enrollment process, you will have full control over where you can get placed and the options available to the participant. The facility is ultimately decided on by the participant.

No. Having Medi-Cal benefits allows you to participate in the program but does not mean that a participant is auto-enrolled. You still need to apply with a Care Coordinating Agency (CCA) to be enrolled.

  • Age 21 or older;
  • Have full-scope Medi-Cal eligibility with zero share of cost;
  • Have care needs equal to those of Medi-Cal-funded residents living and receiving care in Nursing Facilities;
  • Willing to live in an assisted living setting as an alternative to a Nursing Facility;
  • Able to reside safely in an assisted living facility or public subsidized housing;
  • Willing to live in an assisted living setting located in one of the following counties providing ALW services: Alameda, Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, and Sonoma counties

Enrollment Process

Pre-Eligibility

Complete and submit the application packet to determine if you meet the pre-eligibility criteria. If all pre-eligibility criteria are met, GrandCare Assisted Living Services will assist you with the enrollment process for ALW at not cost.

Waitlist Process

If you are residing in a home setting or assisted living setting you will need to be placed on a waitlist. The completed waitlist form is submitted to DHCS. Currently, waitlist applicants are averaging 24 months before approval to conduct an assessment for the applicant.

Assessment

If you are in an institutionalized setting, you automatically qualify for an assessment given that you meet all the pre-eligibility. Once approval is received, an RN evaluator will schedule an assessment. Once the assessment is completed, it may take 1-2 weeks to determine eligibility.

Review & Approval

If the applicant is found to be eligible, we will submit the application to DHCS. DHCS may take an additional 30 days to complete the review and approval of the waiver application. The approval has a 6 month expiration period from the date of approval.

Transitioning to Facility of Choice

The transition can take place in several ways. The facility of choice may offer transportation within their radius of travel. Participants may need to arrange their own transportation.

Ongoing Care Oversight

As the Care Coordination Agency (CCA), we will continue to monitor and oversee participants’ care throughout their enrollment. A nurse or social worker will visit on a monthly basis to check-in and ensure participants are receiving adequate care at the facility.

We will assist you with the entire enrollment process and participation of the ALW program.

The Assisted Living Wavier was created by legislation to provide assisted living care services as a Medi-Cal benefit.
